typically, the first interaction with a new is over the phone. The potential client describes what prompted him or her to contact me. I ask about medical issues, goals and circumstances in order to evaluate whether I consider myself to be a good match for the client (for example, I do not train teenagers and will refer out) or whether our schedules even permit training. We then make an appointment to meet and I send a contract which has questions about medical and exercise history and lifestyle.
When I meet the client, I use the answers from the contract as a starting point to answer more questions.

I use the consultation forms from ACE who certified me. I also use forms from IDEA to add to the ACE forms. Like Karin says, I speak with potential clients to see what they want and need and go from there. The potential client does most of the talking which answers my questions most of the time and leads to questions later that go with the flow of their wants and needs. I charge $50/hour for personal training. You will want to do your homework to find out the going rates in your area.
