I also do not have a favorite, for the same reasons given by Karin Singleton. I customize every journal to the client. Of course, I have the basic format from client sessions which is a head start to alter for the next client.
The SOAP Note, in the ACE Manual, is the only format I do not alter.

I have created fitness journals for clients in the past and usually tailored it toward their specific needs; they often included space for keeping a food journal. I never saw a commercially available journal that was jut right because there was always something missing or it included a category for which I had no use.
Journals can be more tilted towards strength training with the ability to record weight, sets and reps with limited space for cardio. Others can have cardio as a focus with the ability to record whatever measurements are available for machines or outdoor activities.
If you try to cover it all, you end up with an unwieldy construction. The best way would be to include a lot of ‘user-defined’ fields.
