I have been trainin with women who is pear shaped. Her trouble area and most unconditioned part of her body is legs & thighs. Our workouts are focused on leg/butt. We do alot of cicuits w stregthening, cardio & now plymetrics for her lower body. I train her at her home 2x a week. She lost 5 lbs (goal is 10)and lots of inches everywhere except for thighs (only 1 inch). Its been 3 months.
As I am sure others will tell you, it is not possible to pin point an area of the body to shed fat from, the best way to approach this situation is through a mixture of a fat burning type of cardio (ex: elliptical at moderate intensity for 45 minute to 1 hour) and lower body strength training (focusing on compound exercises such as lunges and squats)
As a person with Hypothyroidism I emphathize with this woman as one never knows from day to day how they are going to feel.
With that being said, I am assuming she is being treated for her thyroid.
I would suggest, if she can tolerate it, HIIT in small doses, even 30 second intervals until she adapts
Food intake is also very important with both of her medical issues!

I can see how this would be difficult for you and your client. With that said I have had a similar case before with a client a few years back. what I did find helpful for client to loss the last few kgs and got them healthier was that i place them on a auto-immune diet first.I also kept the training to full body instead of just the spot training as that yielded no significant result for us, with a bit of time saved at the end of each session relaxing our mind and body with some mediation.
