If you run a simple google search you will find a ton of templates. Just double-check and make sure that it includes everything you want in the proposal.

I have 23 years of management experience,commercial , country club and private studio.Your proposal should have specific content as to what you will/want to offer the members.You need to recognize every market and their needs( age appropriate programs) .You should already have current knowledge of the members and the dominant age groups(at my club 55%of membership was over 60 )Also know the GM and his thoughts and perceptions of a fitness center.Is it going to be a profit center for the club?Is it going to be an amenity that is offered free to members and they pay extra for your services?Lots to consider ,be glad to help in any way,good luck.
