In utilizing forms including: Medical release, risk assessment, waivers/liability release you are going to want to consult with a lawyer before implementing such documents. Reason being is that each state differs in the enforceability of different documents. For example, Virginia does not allow waivers/liability releases. Therefore, they are not admissible in the court of law.
