You can certainly find templates both online and from other sources, but I would caution that because this is a legal document, that whatever you decide to use that you have your OWN attorney at least review it before using it. If you attempt to draft your own agreement first and then take it to your attorney, rather than have him or her draft the agreement from scratch, you should be able to keep your costs down. Taking the proper steps to get this, and other documents that impact your business, drafted or at least reviewed by an attorney will help protect you and your business, and allow you to sleep better at night 🙂

I recently signed a form as a subcontractor to a studio and would not mind to send you a copy of it. There is nothing simple about that form, though. It was drawn up by a lawyer.
