i need a good simple liability form

not sure what you are looking for. Liability waivers are usually forms where class participants sign that they won’t hold you liable if they get hurt. I do not understand this ‘affiliation with your local facility’.

You can use a waiver/release I have on my site. copy & then alter it as you like. You can also consult with your professional liability insurance provider. They would probably be happy to give you one of their templates you can use, because it will helps prevent law suits they may have to cover.

If you are an independent contractor I would seek the advice of a lawyer to be on the safe side. Otherwise, your facility should have provided you with one if you work there as one of their employees. You can also check the link below:

The ACE site and texts have forms you can use as a template. You would want to get it altered for your specifications by a lawyer.
