How do you track each of your client’s training sessions?

I have a Printable Trainer’s Note sheet I have created with Excel that shows exercises and workouts plus my own personal notes for the client/session; I keep these on file at home.
I also transfer new exercises completed to a Summary Sheet for the client; I keep one copy and give another to the client. The Summary Sheet provides the exercise with details for proper form, tips or variations along with the current recommended sets/reps/weight.
