forms
If you are an IDEA member there is a icon for “professional forms” on your home page. It is in a group of icons on the right hand side of your home page.
If you are not an IDEA member (you should be), you may be able to find forms on the website of your certifying organization (like ACE, NASM, etc.).
If you are not certified, what forms do you need?

Hi Amy,
I like the brevity of your question. It has a certain mystery to it.
I created a bunch of forms for my yoga teacher business including:
Student Attendance Form
Student Feedback Form
Yoga Questionnaire 1
Yoga Questionnaire 2
Yoga Student Waiver Form
They are all available as downloadable templates within the Yoga Teacher Lesson Plan Kit (see bonus no.18 at http://georgewatts.org/yoga-lesson-plans/).
And if you’re a pilates teacher, you might like The Pilates Teacher Marketing Kit available here: http://pilateslessonplans.co.uk/pilates-marketing-kit/.
The Pilates Teacher Marketing Kit has the following forms:
Pilates Instructor Student Questionnaires
Student Attendance Forms
Student Feedback Form
Student Folder Cover
