Hi Denise. I’m assuming that when you say “in-house personal trainer” that you speaking about a trainer who is an “employee.” If I’m correct in that assumption, then the employer or facility that you’re working for will usually provide the necessary liability waivers and other paperwork necessary for you to train its clients.

If I’m off base with what I’m assuming that you are asking here, please clarify and I’d be happy to give a different response.

I hope that this helps.

Hello Denise, If you’re AFAA certified…you will find an “Agreement and Release of Liability” form in your AFAA book (first edition, 2006, revised 2007) in the Appendix C…page 429.
Hope this Helps!
Sincerely,
Michael

Hello Denise!

I created my liability waivers by integrating the waiver from my Fitness Insurance Company (FitPak), from ACE’s recommended waiver and a little help from an attorney friend now client.

Always remember to cover anything that you may not expect. Kind of like the Warning on a hairdryer by stating… “Do Not Use Hairdryer in the Bathtub or Shower.”

Kindest,

J David Mychael

Denise, as David mentioned, you can buy all the forms from ACE’s website, or what I did, which is time-consuming at first, was create my own. I read quite a few examples online, then made my own based on the different ideas that I saw. Like I said, a little time-consuming, but I’m pretty confident it covers everything I wanted it to cover. At the end, I just have the client and myself sign and date the form, and give the client a copy if they want one. Hope that helps!

I also used ACE and AFAA as templates and then had them reviewed by an attorney. It was worth the money to be sure that I was covered and following state requirements.

Good luck!
