Hi Robert,

If you are a member of IDEA, you can find the PAR-Q on your main page when you log in initially. It’s one of the blue boxes, and it’s labeled Professional Forms.

Yes, and there are a few out there. You can also do an online search to find one that fits your services. Always have legal professional to review it before having clients using it.
