Are there template contracts for personal clients out there or ones that you can recommend to be modified?

Have you tried searching for personal trainer agreements online via reputable associations ACSM, NSCA, NASM, ACE?
Seems to me I have seen sample agreements. I designed my own over 19 years ago. I suggest you take some fundamentals from available sources, and include things pertinent to you and your clientele.
