I am looking for a standard liability waiver form for new clients. I do not work in a gym. I work in client homes, so standard gym facility release forms are not appropriate for my clientele. Does anyone have a good template to use?

There are waivers provided here from Ideafit. They work great for in home clients.

thank you both for your responses. Unfortunately the forms under the personal-trainer-forms link are for trainers who are working in gyms, not in private homes. That is the problem. I cannot find forms appropriate for in-home training.
