If you are asking about the necessity of client forms, then yes, you or your facility should have client intake and liability waivers. At my facility, we also have exercise cards for our private training clients so that we can track progress and if ever a sub is used, they know the clients current level/exercises.
Hi Brandi. Yes, there are several places where you can find client forms and templates, but the one caution that I would give would be please, please, please never simply just start using forms that you get from somewhere else without taking a couple of important steps. First, read them to make sure that they make sense for YOU and your business, and second, if you’re planning to use them, take whatever forms you decide to use and set-up a short meeting with an attorney to make sure that they protect you and are applicable to your business the way you want them to – this can save you a lot in the long run!
