-Would they reccomend you?
-What did they enjoy?
-What did they not enjoy?
Have you come across a form like this? I have made my own but am hesitant to use it. I was hoping to find one that was a little more professional.
Hi Bryant. I use a questionnaire as part of my new client ‘in-take,’ and although I do not use a client-ending form as you suggest in your question, I think that it’s a great idea. In most instances, my client and I have established enough of a rapport that throughout our working together we talk freely about our progress, their expectations etc. However, the idea of making this a more formalized process (especially in terms of soliciting possible referrals) is a good idea I think. To your list, I would probably also have a question such as “can you provide a short blurb/testimonial on our work together…” So often, we as trainers have a great working interaction with a client and fail miserably when it comes to utilizing those great relationships that we build by not simply asking our clients for 1. referrals, and 2. testimonials I think that you’re on the right track with your questionnaire.
