Client ending questionnaire
Do you give anything for your clients to fill out when they are done with you? Asking
-If they achieved their goals?
-Would they reccomend you?
-What did they enjoy?
-What did they not enjoy?
-Referrals
Have you come across a form like this? I have made my own but am hesitant to use it. I was hoping to find one that was a little more professional.

particularly when you are working with individual clients, what do you want to get out of this questionnaire? For the sample questions you list, you should know the answers as part of your constant interaction with your clients. Chances are, if there is something they did not like, they will not write it down if they have been hesitant to tell you.
I see from your profile that you are a trainer with limited years of experience. The way you could approach it is by asking them to help you become a better trainer and giving you feedback. People are usually reluctant to state anything negative but that’s exactly what you need to know. If you approach it like that, I believe you will have a better chance of an open conversation. Better still, ask clients at the start of the training for such feedback, and you will have a chance for a course correction and may be able to maintain the client longer.
