Checklist items for starting my first PT job as a contractor in small gym and in home sessions.
Small gym contractor and in home sessions. Getting ready for first sessions. Am I forgetting anything?
liability insurance, outline for procedures/first time sessions, payment processing, business cards,
Proper clothing, cheat sheet for common HR guidelines, clipboard, stopwatch, legal forms, HR monitor, stethoscope, folders and paper pens, expense log, equipment, first aid items AED.
Anything else I might need? Any extra advice for first time trainer would be great. I feel confident, but extra info never hurts.

you really have been giving this a lot of thought, and your list looks great.
If your first session is an assessment, I would also add a method of determining body composition or at least a tape measure. I also tend to check the blood pressure, and for that you would need a cuff.

Believe it or not, I keep crutches in the car.
How about a locking briefcase for the confidential records?
Do you have a travel log for tax purposes?

Marketing, marketing and Marketing. You can be a great trainer and have all the tools to do the job but you need to familiarize yourself with marketing yourself. Facebook is huge and you need to see what your gym will allow you to do with inside the gym marketing. Keep asking people not associated with fitness industry if your marketing would catch their eye. I am constantly learning and always keep an open mind on advice. Google personal training for your area and see what comes up. There might be a service that you can join that has a high ranking for your area. Create a website and business facebook page. Keep up with social media post that can help people be fit. Just throwing out some ideas, hope it helps.
