All of the above. The ‘why’ is because of overuse from the repetitive motion of pitching (the act of pitching/throwing has been termed by some researchers as one of the most ‘violent’ acts in sports; as a long-time pitcher myself, I can attest to that statement). This is the primary reason why athletes of all kinds need to be proactive (rather than reactive) in performing a strength and conditioning routine specific to their sport before injury occurs.
