There is a website that has an app called styleseat. It was originally designed for stylists, but there is a section for personal trainers also. It works pretty well and clients can get online and schedule appointments.

Getting a Certification that is NCCA accredited will make you far more employable. I'd say at least 90% of your better facilities at this point require that you have one of those certifications to be employed as a personal trainer with them. With some clients I have had them purchase a fitbit activity tracker. Then I friended them so I could look at their daily activity and very active minutes to get a good idea of how much activity they are getting and what kind of activity.

My first guess would be a nutrition issue. It could be too many calories in or far too few. I would have them see a Registered dietitian to carefully analyze their eating habits.

Depending on how severe his condition I would reccommend reffering him to a clinical exercise specialist or physiologist.

That is a loaded questions. Each offer a variety of certifications. Other than the basic personal training certification many of the certifications offered by the ACSM require a degree in Exercise Science or something related.
