Have you checked out PTontheNet or FitnessBuilder? Both have ways for you to manage this information on your iPad. I also keep a file folder with the same information, minus assessment, session plan, etc in case the technology fails.

I require payment up front as it demonstrates client commitment. Decreases chances of cancellation and if they do cancel, I already have payment. Reduces administrative costs and time, benefits I pass on to the client. And its a written policy for working with me.
