Best advice I could give you is to contact her doctor and then come up with a plan. There is also a lot of information online which you can refer to and with some extensive research you can come up with a functional plan. Also talking with a PT might be a good idea as well. I hope this helps.

Although it's completely up to you, a 70-30 or a 60-40 is not unusual. The other option is to charge a monthly fee and take the % per session out of the equation (sometimes this works better).

I agree with the others. It depends on your budget and the type of equipment and amenities you want to put in. You can go around and visit other small studios (or larger gyms) to get an idea of what to buy and as far as layouts. Of course an extensive online search could be as productive. Good luck!
