I would check with your local YMCA (if you have one in your town) and/or community centers. I'm not sure what the regulations are in Canada for minors to be allowed in the gym floor, but you should check on this as well when looking for a gym. I hope this helps

Best advice I could give you is to contact her doctor and then come up with a plan. There is also a lot of information online which you can refer to and with some extensive research you can come up with a functional plan. Also talking with a PT might be a good idea as well. I hope this helps.

Although it's completely up to you, a 70-30 or a 60-40 is not unusual. The other option is to charge a monthly fee and take the % per session out of the equation (sometimes this works better).
