According to an article from Fortune Small Business magazine, reprinted online by CNNMoney.com, a Midwestern hospital operator assesses $5 per paycheck in extra insurance copayments for smokers. In 2009, the company will extend the penalty to workers with high cholesterol, blood pressure and blood glucose. Obese employees will be expected to pay $10 extra.
