Where to find proper Liability Waivers and other Documentation
I have officially started my own training business and would like to ask if anyone would know where I might find proper documentation for free or a cost. when i worked for a health club Liability waivers, PAR Q's, medical clearance, and other forms were always available. where might i find these forms for myself?

If you're AFAA certified...you will find over 25 "Personal Fitness Trainer Forms, Questionnaires, PAR-Q's , Liability and Assessment Forms" in your AFAA book (first edition, 2006, revised 2007) in the Appendix C...pages 418 thru 456.

However, the Liability form did need a little "tweaking" for my state. I had an attorney look at it and add some required verbiage. I wrote a personal training agreement on my own using the agreement from the gym I go to as a template. The attorney also reviewed and tweaked that. He only charged $90 total.
