If you're AFAA certified...you will find over 25 "Personal Fitness Trainer Forms, Questionnaires, PAR-Q's , Liability and Assessment Forms" in your AFAA book (first edition, 2006, revised 2007) in the Appendix C...pages 418 thru 456.

Nothing is too probing when you keep everything confidential; the information will help you help the client. The clients do not have to answer what they are not comfortable with, anyway.
ACE text books also have forms. Whoever certified you should have forms for you to use, as well.

I just did a quick Google search and was surprised how many I found. All I entered was 'personal training health questionnaire'. Here is one example which is very similar to the form that I am using. http://www.evolutioniwc.com/images/pdf/health.pdf. You will also see sample physician approval forms.

Any certifiying agency usually has samples of health, nutrition and exercise questionaires that you can use. Some will even allow you to copy the exact form for your use.

I vote PARQ. Yet its all what you want to shoot for as a trainer- you have loads of papers to choose from, just have to find what fits with your style.
