What risk management protocol do you utilize at your gym or with your clients?

The use of an Agreement that outlines the details of our relationship as trainer and client. A health history, and fitness assessment. A PAR-Q form. A Physician Approval to Exercise form (where appropriate). Not taking on someone who I do not believe I am qualified to train. Full disclaimer/disclosure that I am not at any time giving the client medical or nutritional advice (as those terms are commonly understood and defined)... There's much more that can be added here.

Same as LaRue. Also an Informed Consent. If I have any doubt, even if the situation is fully in the scope of my practice, I ALWAYS require a pysician's approval and communicate with the doc or the physical therapist after the client gives permission to such a dialogue.
