Hello Andrew, If you're AFAA certified...you will find an "Agreement and Release of Liability" form in your AFAA book (first edition, 2006, revised 2007) in the Appendix C...page 429 and 24 other Personal Trainer and Assessment forms.

Get the forms and make copies, no matter what the shipping is it looks more professional to have an organization like ACE's forms for your clients. You could also create your own if you are worried about money but this is not something that you want to skimp on in my opinion.

I wrote my own and it was a pain. Now I use the ACE forms along with my logo and their's. Look pretty professionally if I do say so myself.
