The body needs good quality carbohydrates and proteins as well as fats to function optimally in the long term. Everyone is somewhat different regarding ratio, - however if you're active it is important to not restrict carbohydrate. Make sure they're coming from wholesome, whole food sources.

Both are very important! eat carb's that are high in fiber and have a low GI and eat protein that is clean and low in fat.

Both are necessary for good health but carbs should be the majority of your diet.

Depends what kind of activity. Endurance activities require more carbohydrates and strength and power activities require extra protein. Also if you have a fast metabolism your body might need extra unsaturated fats to serve as fuel to protect your lean body mass from getting used up for energy. It can be complicated to find a balance and it is better to have a Registered Dietician to give you a complete solution and adjustments as needed.

Without knowing about any "special" dietary needs or health issues, I would refer to MyPlate.gov for general guidelines.
