Assumption of Risk & Liability Waivers & Personal Training Client contracts - are there good templates available?
I am looking for:
1) Assumption of risk & waiver of liability forms (at my home studio, in client home, on another business site & outdoor)
2) Personal training client contract agreements (with session tracking),
3) group personal training programming agreement forms, and
4) Medical release forms
Looking for comprehensive examples and/or completed forms I can purchase, or use as a template for my new personal training business.
Any help is appreciated: (actual forms, names of experienced attorneys, online tools, etc)

Hi there~I would absolutely hire an attorney to write your contract and waver, rather than using a template. I did and realized there were pertinent things missing in the one I was using.
Not sure who you are certified through but there are several computer fitness programs and certification organizations who offer free Physician release and Medical release forms.
As for client tracking sheets (for sessions and groups) I've made my own spread sheets to also keep track of payment.

One more thing Jennifer- If you are doing sessions in a "series" I would put in print what happens to money on account in cases of death or disability. It does happen.

If you're AFAA certified...you will find over 25 "Personal Fitness Trainer Forms, Questionnaires, PAR-Q's , Liability and Assessment Forms" in your AFAA book (first edition, 2006, revised 2007) in the Appendix C...pages 418 thru 456.

Hi Jennifer. I totally concur with Kimberly! If I were you, I'd use any template documents that you may find through the various sources as just that - a template that you can then use to sit down with an attorney and have edited to suit your particular needs. While using someone else's forms is a great short-cut, I think that it's potentially dangerous to simply use any important legal document such as contracts, waivers etc. without having a attorney - local to your jurisdiction - review them after a consultation with you. That way, you can help ensure that the forms are actually tailored to your specific business and business practices.
