Anyone use a quality assurance form to follow up with students? (Not evaluation)
I would like to follow up with students who do not return to classes as part of quality assurance for our group fitness classes. I think calling or e-mailing them using a standard form to answer questions about why they have not returned. (Schedule change, didn't like the class, etc.) I would like to track this. Anyone know of any sample documents or suggested questions to include? I don't want to forget anything! Thanks!
I use Word and enter the answers as I get them. I keep it simple and direct.
It's easy to create your own form and add as you go.
Also, I have been taking classes at different studios and none of them have asked my opinion!
You can also do the free online surveys, but they are impersonal.
Let the client co-create with you the experience that will keep them coming back.
